What we are

• A treating provider for injuries from car and other motor vehicle collisions.
• Downtown Oakland. The office address is at the bottom of this page.
• English, Spanish and Russian. Dr. Ben Glass examines and explains in English and Spanish, and Dr. Irina Velichko in English and Russian, without an interpreter.
• Same-day evaluations for new accident patients.
• Auto insurance, MedPay, PIP, and liens accepted.

What our records include

A patient file is organized in this order. This describes the structure of the file; what any one file says depends on that patient.

• Intake history. How the collision happened, each symptom and the day it began, and relevant prior history.
• Examination findings from the first visit.
• Validated outcome measures, recorded at intake and repeated at each re-evaluation.
• Visit notes for every visit.
• Discharge summary at the end of care.
• Narrative report, prepared on request.

How to request records

1. Use a signed, HIPAA-compliant authorization for release of the patient's records, signed by the patient or the patient's legal representative. Say which records you need, including whether you are asking for a narrative report.
2. Send it to the office by mail to the address at the bottom of this page, or contact the office for secure submission.
3. Turnaround is confirmed when the request is received.

Please do not send health information or records through this website. The site has no upload form, and requests are handled only through the office with a signed authorization.

How to refer a client

Call the office. Your client can also call directly. We schedule the first visit and check the client's coverage before it.

What we don't do

• No expert-witness engagements. We are a treating provider, not a retained expert witness.
• No payment for referrals, in either direction. We do not pay for referrals, and we do not accept money or anything else of value for them, from attorneys, other businesses or patients.
• No case evaluations. We do not assess liability or claims. That is the attorney's work.

California Business and Professions Code § 650 prohibits offering or accepting any compensation or inducement for referring patients.
